2025 Agency Agreement
The 2025 Agency Agreement is the starting place for new HMIS end users- simply have your authorized agency representative (anyone who is authorized to sign on behalf of the agency) submit this agreement for staff members who need access to HMIS.
Freshdesk (HMIS Ticketing System)
Freshdesk is our all-in-one ticketing system. Whenever you encounter an HMIS or CES error, please submit a ticket here. The Freshdesk homepage also contains our search bar with our built in report finder, and various help articles to guide you through HMIS processes.
