What to expect
The CES Assessment uses a standardized tool to evaluate the needs and vulnerabilities of those entering the system, providing a fair and consistent method for determining who needs the most assistance. A provider will conduct an assessment to thoroughly understand your current situation and needs. This assessment enables us to identify the most appropriate services and resources for you. Assessments are required for all households that wish to be placed on a regional list for services.
Please know that you can also be assessed and added to CES lists via your current service provider if they are a CES access point. CE access points (access points are CES participating agencies; meaning they can conduct the CES assessment and attend regional case conferencing meetings to advocate for their clients.). Access Point can be found at the bottom of this page.
If you do not have any service providers in your area, fill out a CES Help Request form to submit an assessment request to KSHC. When this request is received, the CES team will reach out to schedule an assessment with you.
TO STAY ON THE LIST, contact an access point in your area or the CES team; service providers with access to the CES database can perform a “check-in,” which restarts the 90 days of inactivity countdown.
If you have received a referral, the service provider you have been referred to will reach out to begin the program enrollment process.
